Online Competitive Event Interest Form:

- will be used to register for the online events
Email us or give the form to us on the due date. Once all forms have been processed and the fee shows up in your account, pay $95 to the bookkeeper under "HOSA Competition Fee" and email a photo of the receipt or hand it to us during club meetings.

- will be used to determine final cost of SLC and the confirmed team roster
